£36,780 - £41,669 per annum pro rata
2 x 22.5 hour per week roles available (2 x 0.6 WTE)

About the role

Research shows that at the end of life and when grieving, what most people need is connection, information and support, our new wellbeing and community support service will be focused on delivering practical, spiritual, emotional and holistic support to patients and families. The new service will also be responsible for caring for staff and volunteers’ wellbeing.

Our National Social work leads will play a very important role in this service by providing an online or phone based (where online cannot be offered) specialist social work service to patients and their families who are supported by the Sue Ryder local palliative care services with responsibility to support the local MDTs on a case-by-case basis and to get service users the support they need.

About you

This is an exciting new opportunity - We are looking for 2 part time qualified and registered Palliative Care Social Workers who have great IT skills and the ability to meaningfully connect with people using online and phone-based technology.

You will have experience of working in the grief and loss space supported by the modern principles of palliative care, loss, grief, and bereavement and apply a public health approach to your work. You will also have great measurement and data management skills to be able to demonstrate the outcomes of your work.

Key skills need for this role include:

• The ability to network, liaise with and partner with other pre death support services in the localities via digital platforms.
• To work collaboratively with members of the local multi-disciplinary teams with screened individuals to assess practical and social needs of service users and of their families and carers.
• To work digitally, in partnership with patients, families and carers by supporting them to remain as independent as possible and to equip them with the skills and tools to live well.

Qualifications:

• Degree in Social Work or an equivalent Social Work qualification with specialist palliative training
• Registered with the relevant professional body (Social Work England)
• Be a member of or willing to join the association of palliative care social workers
