Job description

We have an exciting opportunity for a Clinical Nurse Specialist in our First Contact team at St Joseph’s Hospice.
We are looking for someone who has experience in telephone and in person community assessment and working in a palliative care environment.

The First Contact Team is a single point of access to St Joseph’s Hospice services and is part of the 24/7 Advice line for healthcare professionals, patients and their families. The Clinical Nurse Specialists in the team provide initial telephone assessments with patients or healthcare professionals and emergency home visits if required, with sign posting to hospice or other services, and the provision of specialist information, advice and support. The role involves triaging referrals for most Hospice Services including Inpatients, Rehabilitation, Community, Outpatients and Day Hospice.

About you

You will need:
• To provide expert assessment of patients with palliative care needs who are referred to St Joseph’s Hospice and make appropriate decisions based on clinical judgement
• Assess the appropriateness of the referral and the urgency of referrals received by the hospice
• Appropriate degree level qualification
• Advanced communication qualification
• We would also consider someone with transferable experience who would be willing to start in a developmental role
