Harlington Hospice, St Lukes Hospice, Meadow House Hospice, St Johns Hospice and Royal Trinity Hospice all provide care in the North West London Integrated Care System (ICS). They worked together to develop a collaborative approach to data sharing.

They collected demographic and clinical data about 20 consecutive admissions in each hospice on a specific day in January 2023 (this gave a cohort of 100 admissions in total).

They also asked two qualitative questions:

  • What would have kept the patient at home?
  • What alternative settings could safely have cared for that person?
