This post is an innovate, dynamic, autonomous role to support the development of the Virtual Ward Project situated in the London Borough of Bromley. Health & Social Care organisations in Bromley have agreed to work together to enhance and improve the range, quality and effectiveness of services available to local people. Working as a single system, the One Bromley Local Care Partnership intend to develop a strategy in common and jointly direct the resources, skills and assets available within Bromley in a coordinated way to achieve better outcomes. This service will provide care, monitoring and treatment at home to patients who otherwise would have been in hospital. The purpose is to either prevent avoidable admissions into hospital or support early discharge out of hospital.

We are looking for an experienced community nurse with palliative care experience to help us develop, and participate in, the Virtual Ward delivering a collaborative, integrated model of care that delivers a range of monitoring, interventions and care in the community to avoid and reduce hospital admissions.

St Christopher’s CARE, provides learning and development to the communities we support, and we have academic links with a number of universities, nationally and internationally. This is an exciting opportunity for an experienced community palliative care nurse to join a new initiative within the St Christopher’s Bromley Community Palliative Care Team.

Hours: 22 hours per week - 12-month fixed term contract with additional opportunities available if you want to work full time 

Salary: £46,836 - £52,849 pro rata (inclusive of HCA, salary dependent upon experience)
