This award recognises the achievements of a specific hospice team working together to improve outcomes and deliver exceptional results.

2019 Winner - Wigan and Leigh Hospice

A successful partnership between Wigan and Leigh Hospice and nursing homes in its area has helped to reduce inappropriate hospital admissions for people approaching the end of life and has increased the number of people dying in their place of choice.

Nursing home staff attend training at the hospice and their knowledge of the organisation has increased, with many aiming to pursue a career in palliative and end of life care.

The Hospice in your Care Home partnership has helped significantly boost knowledge and skills in advance care planning in nursing homes due to the training. The number of advance care plans has risen from 13 across eight nursing homes in 2015 to more than 100 to date.

The role modelling provided on a hands- on basis has restored pride and motivation among nursing home staff who have become more confident in caring for dying people.

The partnership has also helped to raise the hospice’s profile with the wider community and had a direct impact on fundraising, with the nursing homes engaging in various activities to support the hospice.

"This was an amazing example of cross-partnership working and demonstrates how knowledge-sharing can have a measurable impact on people’s experience of care at the end of life.

"It clearly shows how a holistic approach can reduce inappropriate hospital admissions and has helped individuals dying in their place of choice. This project can easily be scaled and replicated and we loved how the whole hospice team were included."
