For young people with life-limiting conditions and their families, transition from children’s to adult palliative and hospice care remains one of the most complex and least consistently supported journeys.
Our first Transition Conference will take place on Wednesday 9 September 2026 at thestudio in Manchester, bringing together professionals from children's and adult services to explore how we can do better - together.
Whether you work in an adult hospice, a children's hospice or alongside young people approaching transition, this conference is an opportunity to share expertise, connect with colleagues and shape the future of transition support together.
Who is this conference for?
The Transition Conference is for professionals across children’s, adult and wider palliative care services who are involved—directly or indirectly—in supporting young people to transition, with a particular focus on increasing adult hospice confidence, capability and collaboration.
People who might be interested to attend include:
- adult hospice professionals (nurses, doctors, Allied Health Professions (AHPs))
- service managers and clinical leads
- staff who are working to develop transition pathways
- people working in hospices at the early stage of transition work
- transition leads
- family support staff who work with young people
- professionals working alongside young people with life limiting conditions.
Conference programme
The one-day conference will include lived experience insights and explore what effective transition looks like in different contexts and how collaboration can reduce gaps and inequity in care.
Confirmed sessions will look at areas of care specific to young adults including:
- the role of adult hospices in getting involved in Transition
- deprivation of liberty safeguards and deputyship when caring for young people
- effective communication with young people with a learning disability in palliative care
- transitions for people with learning disabilities
- symptom management for doctors and clinicians
- navigating adult social care in the context of young people’s palliative care
- the role of dignity therapy in amplifying the voices of young people
- providing holistic support for young people in palliative care
